Aviation Safety Reporting

Safety is our number one priority at 91猫先生. One of the most important ways to maintain and improve safety in the Aviation Department is to receive feedback from our students and instructors. As part of our internal Safety Management System (SMS), the safety report is the best way to accomplish this.

This report gives students and instructors the ability to report anything that they witness or experience that may be outside our safety standards or has the potential to be a hazard. This report goes directly to the safety officer who will then use the information that you provide to improve the safety of our program.

When filling out this form you may choose to remain anonymous, put your name if you wish to be associated with the report, or put your name while remaining anonymous beyond the safety officer (check box). Pilots who self-report will not be reprimanded as a result of their report but may be contacted by the safety officer if further information is required.

NASA Safety Reporting

Under some circumstances the submission of a NASA safety report is recommended. If you have any questions about the NASA report or wonder if it would apply to your situation then please talk with your instructor or the 91猫先生 safety officer. The NASA Aviation Safety Reporting System (ASRS) 鈥渃ollects, analyzes, and responds to voluntarily submitted aviation safety incident reports in order to lessen the likelihood of aviation accidents.鈥�
